Form Drop Down Does Not Work On Mobile

I have a form with 2 drop downs and just regular text fields. The drop downs are: Choose Language (2-selections) and Choose Location (5 selections). On desktop & mobile only the Choose Language drop down works. On mobile, the Choose Location does not work. If I create another Choose Location and have them both on the form then the first one works and the second one does not. I am not seeing any scripts being blocked.

It’s a really simple form that comes on a Popup window.

If I try and hide the second Choose Location drop-down, the first one does not work. It’s all pretty weird.

Any help would greatly be appreciated.
