Hi Sarah, the plugin I use is at its latest release; I cannot share the content you asked for because the form is totally blocked and nothing happens when I set some change and hit “save”

Following up on this thread. We tested the form by doing following:
- Adding new form
- Adding a new field
- Adding opt-in check box
In our case, the new form does what it is supposed to do. I’ll mark this as resolved. Feel free to open again if you believe we can further help.
