Form example for Super Search

    Could anybody please post a form example. I am reasonable knowledge with normal forms, but I just have no idea what to do with Super Search to create the form. Whatever I add just show as text between the boxes and well I just do not get it.
    But if I have an example I am sure I will figure it out.
