Form Field Information Not Exporting

I’m using VFB 2.7.1 and when I go to export the entries, the first issue is that not all of the fields from the form are displaying as options to export.
Example, I have Full Name, Email, Address, Phone 4 different times because users are registering teams for a golf tournament. I see this information once, not four times.
The issue may be that they’re using the same field name, but I would assume the database would be able to tell the fields apart or some way to know they are different. I want the form to be simple without a large amount of extra details. Everything displays properly when viewing entries, just not exporting.
The other problem I’m running into is that when I do export the one available version of Full Name, Address, Phone, Email they are blank. Even if the duplicate fields were not filled in.
You can see the form here but exporting data right now is pointless because I can’t get any of the golfer information from the form fields. I would have to go into each entry manually and copy/paste the data.
