Form fields

Hi, I bought today events calendar pro. We will also the ticket plugin update in the next month. For now the free version is o.k.
What I am missing is an option to work on the form fields. We need name, mail, phonenumber and address. How can I get more form fields for the mail? Is there a way to customize the send mail to recepient and to customer?
