Form Fields do not work

Data from form fields is not stored or saved.

So we do not have any of the form data available when someone submits a video review, spoke to chat support who indicated this was a minor bug and you had it in the backlog.

It would seem to be core functionality and the only reason to upgrade to pro (beyond the limit on number of reviews).
