We had the same problem on the English version of the page linked but I did work around it by removing the labels and instead using normal text (which works but makes it harder to read the form responses since the text doesn’t appear in emails).

This issue is related to how Jetpack functions. I was able to reproduce using the English version of WordPress. For reference, what language translation of WordPress are you using?

When you created the English version of this page, what did the radio button label say?
Additionally, this may appear correct visually, but when built this way, the label is not correctly associated with the radio buttons. This is an issue for people who may use assistive technology like screen readers, so it’s not a good long-term solution for this issue.

We are running the Japanese version of WP.
The radio button label for English page was same as the current text “Will You Need Pick-up from Accommodation? (only for Ishigaki City, Fusaki, Kabira Area)“
The same issue also occur with the multiple choice boxes “Rental Gear (for Fun Divers)” and”Extras (NOT included in full set)“
EDIT: we’re running a language plugin (polylang) with main language set to Japanese, I’m not sure if that could be why the error appear on the English page too.
Hello @vikingscuba – At this point, I believe the issue isn’t related to the Polylang plugin. Our development team has investigated the problem you reported and implemented a fix. However, we’re currently unsure when this fix will be released. We’ll keep you updated as soon as we have more information. I expect the next version of the Jetpack plugin to address the issue you’ve raised.
