Form is currently not active

I get the message at the top of my preview

Form is currently not active

It will not allow me to send have a form on a page as it says it is not active yet I clicked the button that says activate

right not it is showing to deactivate but it doesn’t do anything I have tried to remove and reinstall the plugin with not luck

How do I get the form to be active so I can add it to a page?
