So if I am getting this right you were using a form and it has disappeared after you have updated MailPoet.
Could you confirm which kind of form it is, and if it is a MailPoet form or generated with another plugin?
It would also be nice to know what is the MailPoet version that your site is currently using.

Hi,
I’m using the Mailpoët plugin. In the back-end I use the Mailpoët widget.
It used to work perfectly for months and now it’s gone (widget is still in the same place).
I am using version 3.71.2

Also, can you confirm if this issue happened after updating MailPoet? Or was it after updating the theme?
