Form not saving

I’m working on a simple sign up form for a client. She has this plugin which just isn’t working .. but of all the varying issues I’m having with it, the most significant is that the form is not saving after I create it.

The form is supposed to be a simple single text Last Name, single text First Name, single text Email Address, and chick box for receiving an optional email report.

I hit “save form” multiple times but it just doesn’t save.

If I can get assistance with that, the next issue I’m having is applying the shortcode to actually display the form.
