Form Not Working & Styling Question

Hi – I just used your plugin to create a form on this page: http://letstalkmovementbuilding.org/join-list/

Here are my 2 questions:

1) The form doesn’t seem to be working. When I submit a new contact it isn’t added to our Constant Contact account.

2) After I submit the form, it doesnt give me a “Thanks for signing up” message. How can I add this?

2) is there any way to make the fields have a larger height and more space between each label & field?
