Form remembers input data

If a person books an appointment and then decides to book another one, the form is already populated with their answers from the previous time. This is different from Chrome’s autofill option. I want it to be that every time the person books an appointment (even from the same computer/browser) they see a blank form that they must fill from the beginning. On which page would I edit the code, and what changes would I make to prevent the form from remembering and filling in the information?
