Form User Confirmation

I have a form on my site, located at http://oaklilyandivy.com/?page_id=23. I would like a way to deny people who enter their student ID # in wrong.

For instance, John Doe’s student ID # is 00001. I want the form to deny him submitting it unless he enters his code in.

I do not want random people filling out other’s names and submitting profiles for them.
