Form verification

I have been trying for months to find a form that can be used by members in Buddypress. The form requires very basic input (name, surname, date, and one or two radio button options). But it must be addressed to another member on the platform. The receiving member can edit and reply to the sender. But the most important part is that the reply must be automatically stored as an entry on the member profile. I need this to allow for workers to submit hours worked, and manager to verify the data. Stored info can be used by the workers later for other reasons, or just as backup. Will it be possible to build something in this line with Form Builder?
