Form with multiple uploads

    Hello,

    Before I purchase, is it possible to add multiple input options on the form? I require the ability for a user to add 4 PDF / images for part of a claim.

    I’d appreciate know if it is possible, and if there is a sample page/site that I could check out – that would be amazing.
