Form Won't Advance

I can’t get the form to advance to the next page. When all required fields are filled, the “Please fill out this field” tag goes up to the browser’s back button and displays there. The form I’m trying to get working is at http://limohawaii.com/limo/reservations/airport-services/.

FYI – I can advance the form with HTML at the bottom of the page. But it nullifies the required fields.

Any help would be appreciated.
