This has not been working since the last update. It was working correctly before the two decimal places checkbox was added.

Thank you very much for using our plugin. Could you please provide the link to the page that contains the form and indicate the name of the number field so we can check its settings?

DISREGARD. A password manager extension in chrome was conflicting with the equation editor. I removed it and cleared cache and Format Dynamically is working.
