We are using the default theme: twenty-twenty-one in the demo, you can give a try swithcing your theme from astra to twenty-twenty-one
We switched to several plain themes and still have the same issues.
any other assistance or suggestion would be helpful?

So we deactivated WP Smart Contracts, uninstalled all themes and additional plug in related to themes. Reverted to Twenty Twenty one and then reactivated WP smart Contracts and it was the correct format.
Now the questions I have how can we KEEP THIS FORMAT working BUT use other themes that are not Twenty Twenty one? We have several other formats we would like to utilize but are unable to because it skews the look/format/display of the page.

With a clean wordpress install and setting twenty-twenty-one with nothing else added
Also you need to select Skin Light 2 in the settings:

So your telling me we cannot have any other template/theme/skin other than twenty-twenty-one with your plug in???
Where can we edit to use anything other than

No, what I am telling is that if you want to see your site exactly as our demo you need to use 2021 theme with Skin Light 2.
Styling and themes are beyond the scope of our support.
In my 1st and several other poats I explained theme and only used your site as an example of the plain format of 3 images across. Never did I
say I wanted your exact demo site.
