format the description field or custom fields

Hi
I would like to know how I can edit the plugin to do the following or to know if there is another way of doing it?

I would like the user to add certain details such as:
Where a photo was taken
Make and model (for some pictures)
Author
Date
Email (but not to be made public)

I can see two ways to do this, use a plugin to add the fields such as NextGEN Custom Fields but then how do i edit uploader to gather the fields and populate them?

or

show a form to collect the data by using text boxes and then concatenate each box appending <br> and place it in the existing description field when the user presses upload.

Any pointers on how I could do either?
