I visited the page sharted by you in this post and I cannot see any kind of design issues there. Menu, header & footer look the same as on other pages of this site. Can you let us know if the issue is resolved? If not, please let us know more details about those design issues and share some screenshots of those. Also, please let us know in which browser you see those issues.

these are the steps taken,
Created the form, published and copied the shortcode to insert in the page https://www.sfoglino.com/prenota
I inserted the shortcode and if I return to developer mode I see it, if I exit Elementor I do not see the form created with Forminator.

To investigate this issue further, can you please perform a complete conflict test?
That is, deactivate all the plugins except Forminator and Elementor. Then check if the issue persists. If the issue is not showing up, reactivate all the plugins, one at a time, and check when the issue reappears. You need to do the same with the theme as well.

As this is a live website with traffic, please create a staging website and perform the test without disrupting the live traffic. A staging website is a copy of your live website in a new directory on the same server using a separate database.

Hi, thanks for the advice, I followed all the steps indicated, from the staging site by deactivating and activating 1 plugin at a time (even the inactive ones) everything works, crazy!!!
I really don’t know what the conflict is related to 🙃
Now I do the same steps on the original site 🤞
I confirm, everything works perfectly and I have no idea what plugin was causing problems..

We are having a similar problem, except that the form shows up like it should for a majority of our website visitors, but not for a handful of them. So far I have not been able to find anything consistent between each of these users that would point to a problem (one is using Edge on both his Android phone and a PC desktop and can’t see it on either of them — he also tried it in Chrome on his desktop and still can’t see it; another is on a Mac using Chrome and she can’t see it — that is the same configuration I have, and I can see it). I am assuming the issue is on the user end since most people are seeing the survey, and many of them have completed it with no issue, but I’m not sure where to go from here. I would prefer not to create another version of the survey for these few to complete, if I can help it. The one using Edge reported that he did try clearing his cache and powering down and starting back up again, but neither of those options made a difference.
