I tested the reported contact page through two mobile devices and with different browsers, while temporarily enabling all magnification and optimization features of the plugin, but I could not replicate the described issue.
Can you confirm if there are some specific settings related to the Forminator plugin or other parts of the application which have been changed in order to eliminate the problem? If so, is it possible to temporarily set them in their previous state, as that would help us to investigate the case further?

I’ve deactivated SG Optimizer settings on that website to try and resolve the issue so the form may currently work.

Worth noting that when I’m signed into the website the forms display, it’s when not signed in that they don’t.
I have replicated the issue, however, once I purged the SiteGround Optimizer cache the forms were displayed as expected. Our plugin has an auto-flush feature, however, it seems that specific modification was applied to these sites that did not trigger the mentioned feature. I tried to replicate the issue again, by changing our plugin’s settings but the forms have not disappeared anymore.
I would recommend purging the cache from the applications dashboards > top menu > Purge SG Cache and testing again. If that resolves the issue, you should flush the cache after every form modification. If not, please update us with the site that does not display the form correctly after flushing the cache.

Flushing the cache does fix the issue temporarily, but I find that within a day or two the form goes back to not displaying until the cache is flushed again.
You mentioned an auto-flush feature though. Will this prevent this issue from happening? If so, where can I ensure this is activated?
The auto-flush feature is triggered upon saving post/page or modifying settings.
Please post a new ticket in our support system once you notice the issue again so we can perform further investigation.
