forms with data export to printable PDF

I am wondering if there is a WP plugin or other solution that can “easily” integrate the following into a WordPress site:
– the client fills out some personal details in a form, like first name, family name, age, address, etc. Basic stuff.
– when finished and the client continues/clicks to the next page, this data should be autofilled into a printable PDF text with the previous data going into specific places in the PDF document.
– the client should then be able to print the PDF.

Been searching now for a while without result and hope to find the easy WordPress way…
