    Please advise on how to build this formula.

    I trust you’re doing well!

    I understand that you need to make a calculation based on a number that a visitor enters into a form.

    You may add a number field and use three different Calculations fields to calculate a total output.

    Add a number field, right after this a calculation field.

    Please see here how to add a Calculation field:

    Open Calculation tab and add an equation:
    (Number field/1000)*626.96

    Where Number field is added from form fields. Please see this screenshot

    Please do the same for two other calculation fields.
