Please know that I could not reproduce the issue on http://ko4tda.com/contact, and here’s a screenshot.

When you get a chance, could you please try your form again in an incognito window? Or clear your browser’s cache? That is to prevent your browser’s cache from interfering with your test. In case it helps, you can find more details on how to clear your browser’s cache in this tutorial.

However, I could not reproduce the issue on:

• Google Chrome: https://a.supportally.com/i/07Y4nq 
• Firefox: https://a.supportally.com/i/UEymch 
• Microsoft Edge: https://a.supportally.com/i/iRoS9e 

I could only reproduce the issue in the new version (17.2) of the Safari browser: https://a.supportally.com/i/KkR9Cj. This is a known bug with this new version, and our developer team is working to fix it.

I apologize for the inconvenience in the meantime and if I can’t provide an ETA when a fix will be released.

However, I’m taking note of this topic, and I’ll make sure to send a message when an update that fixes this issue is released.

I could try your form using Google Chrome and Firefox on an iPhone with iOS 17.2, and I could reproduce the issue:
