frontend submission missing submit button

Hello,

I am using the walden/extra theme and I have a multisite. The form works great on http://aztest.professionalconnection.biz and has the submit button, but on the other sites it isn’t showing up. http://arizona.professionalconnection.biz and http://jiangsu.professionalconnection.biz/

Any idea why this would happen?
