Have you tried:
– deactivating all plugins to see if this resolves the problem? If this works, re-activate the plugins one by one until you find the problematic plugin(s).
– switching to the Twenty Ten theme to rule out any theme-specific problems?
– resetting the plugins folder by FTP or phpMyAdmin? Sometimes, an apparently inactive plugin can still cause problems.
– re-uploading the wp-admin and wp-includes folders from a fresh download of WordPress?
– increasing the memory available to PHP?

I’ve done everything you listed, still no luck.
Anything else I should try? I’ve built other websites on this same setup with no errors, so this is just weird for me.
I don’t have access to phpMyAdmin, but if I did, where would I look for errors?
Check your uploads path in Settings -> Media.
it’s blank. I pasted in the default wp-content/uploads and that didn’t help either.
Do the images upload correctly?
yup, everything uploads correctly. I can see the images in the Media library and add them into pages/posts individually, but the gallery option disappears whenever I try for it.
Is the Gallery tab disappearing from the Admin area? Or is it that your inserted galleries aren’t appearing on your public-facing site?

Have you tried using a different browser? Or even a different computer?
Yup, tried it on Firefox, Chrome, IE, and Safari. Same thing happens on other computers.
Did you upload these images whilst editing the post or page that you wanted to add the gallery to?
Starting from scratch:
I created a new post, typed in the post title and post content, then clicked on the upload an image icon, uploaded the photos and waited for them all to finish uploading. Clicked on Save all changes which makes the Gallery tab appear. Click on insert gallery. Gallery inserted.
Clicked on the Upload an Image icon again and the four tabs appear (From Computer, From URL, Gallery (7), Media Library). I go to click on the gallery tab and it disappears and instead just shows this: http://i55.tinypic.com/opcytj.png
I know I can click on the gallery in the post and it takes me to the gallery editing options, but even there, if I click the From Computer tab to add another photo, the Gallery tab still disappears.
I’m also now noticing the when I click the From URL tab, it’s missing a lot of options. Here’s what I see: http://i55.tinypic.com/hu3cew.png

Try saving the post before you begin uploading images.

Did this ever come to a solution. My experience is exactly the same, with one exception. The “galleries (x)” tab that is supposed to be there in the “add an image” pop-up window doesn’t appear to begin with, nor do the buttons/features that allow you to attach a photo to a post. I can insert a photo in to a post, but not attach a photo. No gallery tab.
