It should be very simple to add a checkbox like that to your submit-review form. You would just need to create a new checkbox type custom field, put no input values and then make the field required. So people will have to check the one box that is there in order to be able to submit the form. You can also add another text type custom field with a link to your terms and conditions.

3- To add new fields required In-Depth Reviews to be enabled. After enabled In-deepth Reviews, within each review the following words are displayed:
Product Name (if applicable) Score :
Review Author Score :
Reviewer Email (if applicable) Score :
Review Title Score :
Review Image (if applicable) Score :
Review Video (if applicable) Score :
Review Score :
Review Category (if applicable) Score :
Privacy Score :
I do not want those words are displayed.

Sorry for the delayed reply (we were closed on Monday).
We noticed on https://www.guidedolomiti.com/libro-ospiti/ that the shortcodes are displaying as text (instead of the submit review form). Did you deactivate the plugin recently?
If so, can you reenable it so we can see what you meant for the reviews you mentioned in point #3 ?
