Getting a 404 Error

Please help me fix my MailChimp install of this plugin. Every time I try to test the form to subscribe, I get a 404 error page.

I’ve watched Carrie’s tutorial video and still can’t figure out why it isn’t working for me. I’m only inlcuding the form action code between the “”. I tried creating a new MailChimp list, just in case, but no dice. Nothing seems to resolve the issue and allow subscriptions to go through. What am I doing wrong?
