hreflang should be added to all the alternate pages. Please see “hreflang error no return tags” on Google for additional information and examples.

The use of x-default depends on the specific scenario. With the plugin you can find x-default as the first item listed in the “Language 1-10” selectors.

So, if I understand it right, then I should add both hreflang to both sites if I have two sites?
For instance, if I have a swedish site and an english site then I should add both sv-se and en-us to both pages?
Like this:
