This is really weird. Try disabling all plugins and see if it still displays just shortcode. If it renders properly, then enable your plugins one by one and see after which one’s activation the form breaks.

I just took an old install of wordpress and about 15 plugins needing to be updated and deactivated all of them. I updated wordpress to 3.6.1 and installed the front-end uploader on the 2013 default theme.

Ok…
I did a complete update of all the plugins. Even though they were inactive. No change as one would think.
All of them were deactivated and everything updated and still no change.
THEN, I decided to just uninstall all of them. As I said before, it was an old install. A ‘playground’ so to speak.
After uninstalling everything and bringing wp down to ‘barebones’…It works. Still not sure if I want to put it on a development site though. We’ll see what happens along the way.

Could you at least name the plugins that you had please? I have several ‘usual suspects’ that break stuff, but try to make the plugin to play as nice as possible with the others 🙂
It would be much appreciated.
