    Agree with JMSDOnline’s review. Unfortunately the form I wanted to put on our site, for camp registration, is pretty voluminous — paper version runs 4 pages. I don’t know if there’s any way to make it usable on a phone, but in this plugin everything seems to stack vertically and there would have been a lot of scrolling.

    So I asked Simon for a refund and it showed up on PayPal within hours!

    I’m staying on Simon’s mailing list for the future.
