    I’ve run this plugin on my website and love it! I’ve been on here before and gotten the plugin working.

    Yet, how can I add form fields to the admin email?

    Anyone else with this issue?

    I am glad that it is a useful tool on your site!
    Regarding your client site, please check in general if emails are being sent? It is probably a server issue and maybe you require some kind of SMTP plugin, like WP Mail SMTP
