@theenic I see your text is on highlight mode. There can be multiple h1/h2/h3 heading tags in a story and individual tags can be duplicated when using the text highlight fill mode. Although some tools may flag this it is expected behavior and you can safely ignore that warning. Please let me know if you have any other questions. Thanks!
