Would you mind providing me with the URL of your site so I can take a look at your current form and do some testing? Additionally, can you clarify for me which email you aren’t receiving?
It sounds like the current form is adding subscribers to your MailChimp list, but in order to do some more testing I’ll need to take a look at the form on your WP site directly. Additionally, the MailChimp List Subscribe Form certainly does allow for different fields other than email address to be included, and those form field settings are available on the plugin’s settings page.

Thanks for sharing your URL. I located one subscription form on the About Us page, and this form appears to actually be using the Optin Forms plugin by Codeleon (http://www.codeleon.com/wordpress/plugins/optin-forms is the link in the source code) rather than the MailChimp List Subscribe Form I can support here in this forum.
That said, when testing your form, I definitely was able to receive the opt-in confirmation email. I’d recommend checking any spam folders, the Promotions tab if using inbox, and whitelisting MailChimp’s sending IP addresses if you’re using a private email domain (http://mailchimp.com/about/ips). Taking those steps should help! If you need additional assistance with the signup process, I’d recommend reaching out to MailChimp support directly. My role here in the forum is specific to this WordPress plugin.

I’ve taken a look at the “GET MORE STUFF LIKE THIS IN YOUR INBOX” form in the sidebar and it looks like this is a different plugin than what I’m able to support here. From what I can tell the form widget there is associated with the MailChimp for WordPress plugin: https://ww.wp.xz.cn/plugins/mailchimp-for-wp/
That plugin would have been connected to MailChimp using an API key grabbed from within your MailChimp account.
Unfortunately I’m only able to help with the MailChimp List Subscribe Form plugin in this support forum, but I’d recommend reaching out to the MailChimp for WP support folks via their forum for further assistance with your sidebar widget.
