help with Gentle Calm theme

I created an archive page template but it doesn’t display the sidebar. And after I created a page template it doesn’t display in the page template drop-down in the admin. Basically, I want to create a page template that displays the same contact that appears on the index (without the creation date that appears on the default template page). Any help is greatly appreciated.
