How can i edit forms?

I am talking when i click to register in membership levels i see 3same forms..

Sorry for bad english but i would like to explain this with photos.

How can i edit those for every level i have becouse are same.

Hello. Thank you for using Paid Memberships Pro. You can display the points as a bulleted list by using the addon found here:

http://www.paidmembershipspro.com/add-ons/plugins-on-github/levels-page-ul-layout/

If your description has an unordered list of bullet points, it will display them on the level’s page. However, please note that they may still have to be styled in order to display how you want. Let us know if this helps. Thanks again.
