How Do I Create an Offset Rectangle Hover Effect?

Hello. I just bought and installed Image Hover Effects Ultimate and I need to know how to create the hover effect here:
I just need a small gray semi-transparent rectangle offset to the left with the title on it to fade in when you hover over the photo and then fade out when you move the mouse pointer off of the photo. I don’t need any fancy animations or anything like that. Can you please tell me the steps to create this simple hover effect using the plugin?
