How do I remove infected files?

    SiteGuarding plugin says I’ve been hacked.
    The infected file:
    /wp/wp-admin/upgrade.php

    Malware Type:
    php.cmdshell.unclassed.356

    File with malicious codes:
    /wp-content/plugins/wordpress-seo/admin/class-admin.php

    What do I do to get rid of these files? Does it mean I should manually reinstall WP 4.0.1? How do I do that?
