How to create custom widget header?

    Hello,

    I’m wondering how I can create a custom ‘header’ for a widget. We’re using a survey tool but I’d like to have the same ‘header’ like the one (categories) above the survey. English is not my native language so I might use the incorrect word but probably you’ll know what I mean.
