How to implement shortcode in my upload template?

    As you can include or a shortcut for uploading files, be done through my contact form. Wish when the student uploads go directly to an email box and not be stored in the media on my site.
    I would like to implement this code:
    <?php echo do_shortcode('[contact-form-7 id="44117" title="Formulário de contato 1"]' ); ?>

    I want to implement the “question-type-upload.php” page template in my child theme, you can submit my form but there seems to be an error because the “Complete questionnaire” button does not work. can anybody help me?
