how to insert .js ?

hi
i want to iinsert dynamique header un my eb site

what i did and it did not work is :
1. i put css in style.css chld theme
2. i put html code using you plugin in header
3. i put the .js file in include using filezila and not you plugin
the name of the .js is ” index.js ”
but i dont see the effect !!!!

and how i put js in your plugin ? do i must put something in the begining ?
