How-To Make application form interactive?

I’m trying to make an application integrated on a webpage. Right now it’s in PDF form and I want it to be an interactive version actually on the site. Now how would I do that. Here’s the application http://tcrfg.com/images/application-en.pdf I’m trying to make it so someone can just click apply and it will take them to a page where they can fill out the form above.
