    When I updated the above page on Aug 11, and tested it by filling in the html form and clicking “Submit” (which goes to https://howardbirds.website/butterflyreport/ButterflyReport_acknowledge.php)

    I get the above error message and the error log 12-Aug-2025 13:59:37 UTC] PHP Warning: Cannot modify header information – headers already sent in /home1/howardbi/public_html/wp-content/plugins/form-maker/framework/Cookie.php on line 72

    I noticed that the page you mentioned seems to no longer exist, so I’m unable to check the gallery directly. Could you please share a link to a page where the Form Maker form is currently embedded to? Additionally, could you please specify the version of the Form Maker plugin installed on your site at the moment of encountering this issue. That will help me look into the issue more precisely.

    In the meantime, have you tried testing for a possible theme or plugin conflict? You can do this using the Health Check & Troubleshooting plugin. It allows you to temporarily disable themes and plugins only for your logged-in session, without affecting what site visitors see. This way, you can safely check whether the Photo Gallery plugin works properly with the default theme and no other plugins enabled.
