HTTP ERROR 500 on Submit

After filling out the customized registration form, I hit submit and receive HTTP Error 500. If I disable the plugin and use the standard WordPress registration form, it submits with no errors.

Any ideas here? Or is this plugin just broke now? Seeing a lot of others having issues…
