Hello! We will definitely get to the bottom of this for you, but at the moment I am having trouble replicating what’s being described here. If you could provide some more info that would be excellent:
1. Your PHP version
2. A screenshot of the error you are seeing
3. Starting at the dashboard page, walk us through the steps needed to get the error you are seeing

Dashboard->Post->Add New->Add Media->Upload Files->Select Files-> error shows up after the upload starts
OK I was able to reproduce and we are looking into the issue now. See #303 in GitHub if you want to follow our progress there.

I’m having this problem too on our site. It costs many nerves to find out that Stream causes this problem. If it helps, here is the PHP error I found in the apache2 logs:
[Sat Mar 08 12:40:25 2014] [error] [client X.X.X.X] PHP Fatal error: Call to undefined function get_the_title\xe2\x80\x8e() in /var/www/wp-content/plugins/stream/connectors/media.php on line 131, referer: https://domain.tld/wp-admin/post-new.php
