I cannot accept new affiliate

Hi!
I installed your Affiliate Manager plugin.
After I received new affiliate aplication and want to accept it, I click Accept but then error occurs and I cannot accept it. Error messege: Invalid state transition. Please help I cant use this at all!

Another question is: can I generate affiliate link for the affiliate and send it to the person via email by myself (not via WP).
