Is anyone else seeing messed up/mis-aligned icons?

See: http://screencast.com/t/r1g211JCi for an example, but every backend page has this issue.

It looks like the gui buttons are really one big long graphic and we’re supposed to see just a portion of it. It looks mis-aligned.

Plugin version: 2.2.5
Tested on:
Chrome: 51.0.2704.84 (64-bit) Mac
Safari: 9.1 Mac
Firefox: 35.01 Mac

If you clear the cache, the icons display persists ?

Same problem here, also problem on mac in safari and chrome. Looks fine on my windows machine wit IE.
