I have one website that is suffering the very same issue for a few days now. However, ONLY in a widget on a loading web page. All other uses of the form and CAPTCHA on the same website work fine.

These queries cannot be edited after resolves are found to remove URLs. Thus, I canNOT post the URL of the website I am referring to here. However, if you can allow me direct contact, I can offer you temporary access in hope of a resolve to this issue. Please advise.
Not being one to sit, waiting for a plugin author to respond with help, I kept on trouble-shooting last night. I came across an issue with another’s CAPTCHA plugin that was detailed here: http://www.gopiplus.com/work/2010/07/18/simple-contact-form/#comment-180804 and sent this URL to the hosting web server to check out, being as they too are using a Linux server.
However, their response verified that all is as it should be, thus, after checking in here and finding no response to these queries, I kept on searching…
That’s when I found this REALLY old cry for help regarding the same sort of browser behaviour on my CAPTCHA images in a widget: https://ww.wp.xz.cn/support/topic/plugin-contact-form-7-captcha-image-does-not-display
So, I checked the .htaccess file on the server and found so much code that I had to paste it into a document that would automatically break each line of text so I could at least try to understand what was happening. That’s when I noticed a potential issue…
The website’s security plugin offers caching. In the .htaccess file it stated that I had to disable caching to remove these lines.
I tried a lower setting for caching but that option made NO difference, so I turned OFF caching… and VOILA! The CAPTCHA in the loading page widget is now displayed properly, once again.
