Question mark generally means Internet Explorer is not finding the image at the URL the code specifies it is located.
Normal debug. Disable all plugins and switch to twentyten theme. Add everything back one-by-one until it breaks.

Did what you suggested, kmesssinger, but the problem persists even with “twentyten” and no plugins activated. The images upload as usual and are visible in the editor (as you can see from the link to the screen shots), but they still appear as question marks in a post.
I installed the Artisteer theme on another site with same plugins activated, but still on the same server, and here I have no problems with uploading images.
Very strange. Any other suggestions?

Which page are the ? marks on. Galleri looks ok as does the home page with your picture in a post and on the banner. That is in IE8.

The problem is with new images only.
I could give you access to edit the site, if that could help to locate the problem?
I guess your email is webmasXXX at XXXexcellentadventures.com
No access needed.
Sorry, sometimes I don’t read all of the posts – what result when going to the image url with the browser?
What is different about these images? Are you sure they are rgb? Jpegs?

All rgb jpg.
I’ve just published a new post to visualize the problem.
larsravn.dk
The code for your new post is looking for the image at
http://larsravn.dk/wp-content/uploads/2011/04/koervel_8513-300×300.jpg
and it is not there. Paste that into your browser’s address bar and see for yourself.
I suggest you first locate where on your server the WordPress upload process placed this image. Where you find it will help in debugging what is wrong.
Use an FTP program to locate the image.

You are right, stvwlf. I’ve made the folder myself in my FTP program, and now it shows up. Doesn’t WP usually make the folders automatically?
yes, if it has permission to create new folders. That is a server configuration setting. Perhaps contact your webhost and see if they know why WordPress couldn’t create a new folder. They may be able to change a setting or give you a workaround.
If it worked correctly before and no longer works they may have reconfigured settings on the server.
