That’s basically… impossible. I think your issue is happening because of caching, and you were tricked to think that it works after moving the files to the Trash.
Removing files from the Trash only removes them from that special directory, so it can’t affect your site at that point. It can only affect it before when the files are moved towards it. I think you understand how it works anyway 🙂 But yes, I think the issue is linked to caching. I never had this case with my users before neither.
Last but not least, please have a look in your PHP Error Logs, there might be something else happening.
Please help me. I am [email protected] and relatively new to wordpress. I did a website for my optician https://wohloptics.com and after using the free versioni over one month ago before the coronavirus, I was glad to see such a plugin that would remove unused images. However, I now find that my USED images were deleted. Do I need to install the paid version to find where they are and recover them. Please help. There are well over 200 that were deleted and I created the images in Photoshop. Thank you so much.
This will take me weeks to re-do all of these images and to add insult to injury, I have a retina pucker and only have 40% of my vision in my right eye so using a computer is very difficult now. Gail Wright

You do not need the Pro version to recover files. That wouldn’t be nice to add that option only in the Pro…
You can activate the “Recover” feature in the Settings of the Cleaner. Then you will have a way to recover them in the “Trash” section of the Cleaner Dashboard. If you emptied your trash, of course, that would be too late.
